Full-Time MDS Coordinator - Grand Junction, CO
Must be an RN licensed in the State of Colorado. Experienced in an LTC environment, minimum 1 year in completion of the Minimum Date Set (MDS), admissions assessment & resident care plans, basic physical assessment skills, function independently throughout the facility, ability to work collaboratively within a multidisciplinary treatment team and proficient computer skills.
